Candidates can get the form by writing in to the address mention below. Send a Demand Draft in favour of 'The Executive Director, Karnataka Examinations Authority, Bangalore' for Rs. 550 (Rs. 400 for SC/ST/OBC and Rs.650 for candidates not residing in karnataka) along with the brochure.

If the candidate is studying in a school in Karnataka then the student will be informed of and given the form for the CET exam by the school however if the candidate is not getting the form from the school/is not residing in Karnataka then the student an get the form by writing in to the address mention below below. When filling up the application form, make sure that you read it at least twice to avoid making any mistakes. Mistakes will disqualify the candidate from the exams. Below are some of the points that need to be kept in mind while filling out the forms:

1. The names mentioned on the form should be exactly as they appear on the 10th and 12th certificates.

2. If any reserved categories are selected then they should be marked clearly and supporting documents provided

3. Choose the venue of the examination as the city you studied in

4. Date of birth should be the same as it is on the 10th/SSLC certificates

5. Make sure that appropriate contact numbers are mentioned. If the candidate does not have a telephone then the student may provide numbers of neighbours/friends

6. The fields, where shading of the answer is required, the field must be shaded completely without coming out of the border.

7. The impression of the thumb on the left hand is a must for all candidates and should be made in the prescribed manner.

8. If the candidate residing outside Karnataka has received his/her brochure via speed post then the student should also send the completed form back by speed post

9. Always take a photocopy of the completed form prior to sending the form

10. Candidates, who sent their forms by speed post, have not received their admission tickets can appear for the examination provided they carry a photocopy of the form they sent and the acknowledgement of the receipt of their form via speed post
